Caregiver

Positions Open:  (3)Caregiver
There are positions open to work as a caregiver within the patient's home and there are both full-time and part-time positions that are available that allow you to have a flexible schedule. 

Requirements:
-Must have clear background
-Must have reliable transportation 
-Must be reliable 
-Training is provided if required

Documents Required: (Please email all below documents to homecare@gracestlc.com)
- Drivers License or State ID 
- Social Security
- Green Card (if applicable) or must be a Citizen 
- Adult CPR card (current)
- Car Insurance OR must sign a waiver
- Voided Check or Direct Deposit Form 

Instruction for filling the application forms:
-Please print, fill in and scan and send it back to our secure email homecare@gracestlc.com or drop off at our office.  If you are scanning and sending it back, please keep the original application. 
- Only use BLACK ink to fill out the application
-If you make mistakes do not scratch out mistakes but strike out with a single line and initial with the correction. Please make sure all areas marked with an 'X' are signed and filled in as needed.
